Wellness Works Professionals accepts referrals from physicians, rehabilitation consultants, insurers, employers, therapists, employment agencies and client representatives.
The following easy steps can be taken to refer to one of our programs:
- Identify the service required. If you have questions about our services, please contact us by phone at 705-740-2135 or toll free at (877) 449-4446.
- Complete the referral form, including the service you are requesting. If you are referring a client for rehabilitation or assessment services, please include the questions you would like us to address and/or the objectives of rehabilitation for the client.
- Send the completed referral form by mail, fax, or e-mail accompanied by any relevant documentation.
- We will contact you if anything needs to be clarified.
- We will contact the client with appointment times and notify you of the same.
Wellness Works Professionals is a privately funded health care facility. Services are provided on a fee-for-service basis.
Clients are funded through:
- Workplace Safety and Insurance Board
- Automobile Insurers
- Long Term Disability Insurers
- Ontario Disability Support Plan
- Extended Health Care Benefits
- Employers
Physicians, if you are unsure of a patient’s coverage, please make the referral and we will investigate coverage.
